  Data Subject Action Request Form

According to the Personal Data Protection Act 2019 ("PDPA"), a data subject is given the right to perform legal acts on his/her personal data, which include requests to withdraw consent, to access personal data, rectification, erasure, suspension of use, portability, object to the processing, of personal data. Therefore, the data subject can exercise such rights by providing these following details to Siam Retail Development Company Limited:

I, hereby, have enclosed these following documents to verify the identity of the applicant and the data subject for the company to properly execute such requested rights.

Documents for verification of authorization

 Power of Attorney must include these following details Download..
1) It must include a statement “Applicant is given the authority to process with the requesting of rights (please specify the right to be requested) to Personal Data of the data subject, including processing other relevant actions until they are completed.”
2) It must contain clear signatures of the Grantor and Authorized Person.
3) A clear date of authorization must be indicated, which must be the date before / the same day that the request is submitted.

Documents for verification of identity

 Copy of Thai National ID Card of the data subject and the applicant (For Thai nationality)
 Copy of passport (For foreigners)
 Other documents issued by the government (If any). Please specify

The company reserves the right to make inquiries or request additional documents from the applicant. If the given details fail to clearly demonstrate that the applicant is the data subject or has the authority to file such request, the company reserves the right to reject your request.

Requests to exercise the right of the data subject are as follows:

If you wish to exercise your rights on more than 1 request, please submit a separate form to exercise your rights to personal data. This is to prevent any confusions for the company and for the benefit of your requests.

Reasons for your Request

The company reserves the right to reject or suspend the fulfillment of your request to comply with relevant laws.
1) You are not able to clearly verify that the applicant is the data subject or authorized to submit the request.
2) Your request is unreasonable, for instance, the applicant may not have the right to erase personal data or the company does not have your personal data.
3) Such request is redundant, for instance, such request is repeatedly submitted or duplicated without reasonable reason.
4) The retention of personal information is based on compliance with the law or the exercise of legal claims. The company is, thus, unable to provide data or to process your request.
5) Your request does not comply with the conditions specified by the Personal Data Protection Act or the company has reasons to reject the process of your request as specified by the PDPA and those reasons are legitimate.

The company will notify the result of your request within 30 days from the date of receiving the request. If the company rejects the request, the company will return your request along with an explanation for rejecting your claim.


Acknowledgment and Acceptance

You have thoroughly read and understood the contents of this request and confirmed that any information given to the company is true and accurate. You have understood that, in order to consider the processing of your requested rights, it is imperative to verify your authority and identity. If you deliberately give some wrong information to the company, you may be prosecuted according to the law. You have confirmed that in the exercise of rights according to this document, you have legal rights and that you have no intention to cause any damages to any person or to the company. Thereby, you have agreed to provide signature as evidence.
